LVN, LPT – Work in a call center atmosphere assessing the clinical status of patients referred by our Healthcare contracts. Through a brief survey consisting of scripted questions, the Screener will assist in the detection of at-risk patients for behavioral health related conditions. If the screening indicates the possible presence of a behavioral health condition, the Screener will be responsible for scheduling an in-home assessment by a licensed clinical practitioner.
**Santa Ana Office: Full Time, Monday – Friday, 8:30 am – 5:00 pm
Assist the member with IT set up for virtual conference/telehealth visits. Send member an email confirmation for their appointments. Call members if rescheduling is needed. Position will not be expected to assess clinically or have any clinical discussions with members. Position will need to be personable as they will represent Windstone’s “caring campaign” during Covid period. Candidate must be PC, tablet, smartphone proficient, zoom, video conference connection
